The Change Foundation announces call for nominations for 20 Faces of Change Awards - Awards will honour local healthcare heroes across Ontario

Toronto, Ontario – November 5, 2014 – The Change Foundation is pleased to announce the launch of their 20 Faces of Change Awards, which will honour those who have inspired positive patient-centred change in Ontario's healthcare system. The 20 Faces of Change Awards will recognize 20 people, teams, groups or organizations whose hard work and innovation have had a major impact at the individual, community, organizational or provincial levels.

"These awards emerge from our mission to support and advance a patient-centred system in Ontario," said Cathy Fooks, CEO of The Change Foundation. "It's a great opportunity to recognize and spotlight people, teams and organizations across Ontario who are working to create a better healthcare system. Good ideas are worth sharing."

Ontarians from all aspects of the healthcare system who have been instrumental in changing the patient experience for the better are eligible to be a Face of Change, including patients, caregivers, healthcare leaders and organizations, family members, community members, volunteers and staff. The awards will be presented at The Change Foundation Capstone Summit in March 2015 in Toronto, and all of the winners will be profiled in local media, in social media and online.

An independent selection committee that is representative of the breadth and diversity of the healthcare system will review the nominations to select the 20 Faces of Change. Those wishing to submit a nomination are asked to visit the official website at www.20FacesofChange.com. Nominations will close on November 28, 2014.
