Aircrew Flight Path to Holistic Health

Air travel, particularly over long distances, exposes airline crews to a range of health hazards associated with a confined and pressurized cabin environment. The Aircraft Cabin Environment and Health Risks
Beyond understanding and adopting air travel health precautionary measures, aircrews require and should be provided with regular wellness breaks to enable them to physically and psychologically relax and rejuvenate. Such breaks should take account of the cumulative and compounding effects of the risk factors including,
Take-off and landings: The often encountered ear popping sensation during aircraft climb is caused by air escaping from the middle ear and the sinuses. As the aircraft descends on landing the opposite occurs. In cases when this air flow does not adjust to the pressure changes, ears and sinuses tend to block and can be painful. Also the risk of injury is high in cases of a-priori ear or sinus infection,
Cabin Air Pressure: At cruising altitude the cabin air pressure is equivalent to atmospheric pressure at about 2000 meters above sea level and in such an environment there is less blood oxygenation that may cause mild hypoxia, hyperventilation, headache, insomnia and digestive dysfunction
Cabin Humidity: Typically in most aircrafts the humidity is at less then 20% (compared to average of 30% in typical homes and offices) and this low humidity can cause dryness of the skin, mouth, and eyes and possibly dehydration. In addition low humidity together with warm air cabin temperatures can also create increased microbial and pathogen infection viability.
Cabin Air Pollutants: The confined space and high passenger density increases the dosage and length of exposure of air pollutants. Carbon dioxide concentration in air cabins generally exceed the 1000 ppm level, especially when ventilation systems are not fully functional as on the ground during taxiing, take off and landings. There is also the risk, especially in older aircrafts, of ozone in the upper atmosphere entering the aircraft cabin together with the fresh air supply. Ozone inhalation can cause irritation of the lungs, eyes and nasal tissues.
Cabin Chemical Pollutants : The pesticide disinfection of aircrafts, especially those flying from, for example malaria and dengue fever endemic areas, as well as sterilization materials used in aircraft cleaning also pose a range of hazardous chemical risks especially as air crews are exposed regularly and over long periods of time. The presence of organophosphates in jet fuel and aircraft lubricants leaking into aircraft cabins can also cause serious neurological effects
Cosmic Radiation: At aircraft cruising levels, the exposure to cosmic radiation is higher. While it is recognized that frequent exposure to cosmic radiation can cause cancers and genetic damage, there has been little research to investigate long-term impacts of this on airline crew health. Airline pilots are also exposed to cancer risks due to intensive magnetic fields from highly concentrated electrical consoles.
Cabin Space: The limited working space and facilities, for example, pushing heavy food trolleys in and out of tight chambers and serving a large number of passengers across rows of seats in a relatively short time contributes to physical and mental stress. Furthermore the limited in-cabin mobility can also result in swelling and stiffness with a higher risk of Deep Vein Thrombosis. DVT risks are also compounded by low humidity and dehydration, reduced blood oxygenation and higher blood coagulability due to reduced barometric pressure. The very limited crew rest areas, especially to comfortably nap and sleep on long haul flights lead to stress, exacerbating the risks of jet lag.
Jet lag: While airline schedules are designed to give crew sufficient rest periods on the ground between flights, many crew members find it difficult to adjust to time changes and thus are sometimes unable to fully rest and revitalize in time for the next flight. There is growing body of evidence that jet lag results in fatigue, stress, reduced alertness and concentration, mental impairment, irritability, nausea and digestive problems, all of which affects the performance and wellbeing of air crews and in turn the passengers.
Passengers; Air crews are faced with a range of passenger problems, including risks of infections and communicable diseases, medical emergencies, alcoholism and abusive behaviour, communication difficulties etc and they have limited time and a very confined public environment to resolve problems and concerns.
